Show which username triggered the "insufficient attributes" authentication error

We occasionally see "Authentication failed - insufficient attributes | Expected unique user id attribute not found" errors in OpenAthens Admin under Auditing. Currently, these error messages do not show which username experienced the problem. This makes it difficult to identify affected users or investigate whether specific accounts have been compromised. Adding the username to these error messages would help us troubleshoot faster and determine if the issue affects multiple students or isolated accounts.
